Usability Testing Company Recommendations

Hey guys,

Does anyone have a website or company they recommend for some down and dirty user feedback? I don't need great depth to this testing nor want to pay a fortune. I would like the ability to specify a few tester demographics such as: male, ages 35-50, moderately tech savvy and considering a car purchase.

Usability Testing Vs. Solicited User Feedback for Web Sites/Apps

The topic of soliciting mass user feedback regarding the usability of web sites and applications continues to pop up for me. Primary stakeholders ask the question, "Why don't we just send a link to a survey in our newsletter to all our registered users?" or, "Let's add a feedback link or a thumbs up/down clicker to our new beta releases."

Best Practices for "Was this helpful?"

Many sites that offer help conclude the information with a feedback question such as "Was this helpful?"  This can be a valuable mechanism to tailor online or in product Help better. 

Our team would like your help:  We'd like to hear what everyone does.  What question do you ask with what scale or input?  How well does that work?  What would you change?  What are some best practices for "Was this helpful?" questions.
