User Feedback form

Hi all,

The project I am working on is currently going for end user testing. But we
donot have facilities of user monitoring through videos or web as such. We
are, as a first step, planning to hand over a form either online or on
paper, which the user fills up after his interaction with the system. I know
this is not advisable but we don't have options over here as of now...

Is there a kind of a "standardized" form available which has proved
effective in gathering the user feedback as accurately as possible? Also, I
was trying to develop a feedback form here. I was wondering if breaking down
of the questions based on the "tasks" they perform is a good idea? Or is it
better to break it down based on the modules as in the requirements
specification?

Any thoughts are welcome.

Hi,

You might want to look into the System Usability Scale (SUS) questionnaire.
It is designed to capture global usability issues with a product, & wherein
users answer questions along a scale ( I strongly agree / disagree & a
couple of scales in between).
