Here are the results! Thanks so much for the great response. One
thing: I wanted 50 responses and somehow SurveyMonkey let 51 through.

Very interesting results. I'm sort of kicking myself for not adding
"contextual inquiry" to the list of user-research methods. Several
people added it in the "other" category. :)
