Hello. Is anyone familiar with a usable open source tool for creating web surveys? It should support some basic IF/THEN logic for dynamic questions.Thanks! Damon
It also has a lot of tools for statistical mathematics and the like. Whether it is usable or not, I'll let you decide. It ain't pretty. that's for sure!
I have used limesurvey extensively also, and while it it isn't pretty, as noted above, it definitely does the job, is themeable and extensible, and provides plenty o'skip logic. It can certainly be made to be very pretty. If I may make a recommendation, bulldogsolutions.com (Bulldog Solutions) has TONS of experience creating custom surveys using this tool, and are great company to work with. Talk to Rob Solomon (CEO) or Chris Parisi (VP IT), and tell them Kelly Bell sent you.
I can also say that 6 months ago I did exhaustive research on this very subject, and limesurvey was the only app I found that was even close to fitting the bill. I hope this saves you some time and effort. Best of luck, Kelly
Wordpress cms have good survey plugin http://www.surveygizmo.com/add-ons/wordpress-survey-plugin/ . I tested some times ago, and was enough on websites.
I've used LimeSurvey in the past, was happy with it though setting up the surveys did take a bit of learning. I didn't require any logic though I think it had some branching.

I had used http://www.limesurvey.org/ for a relatively complex survey (lots of conditionals).
