Web "feedback" button effectiveness

I'm wondering if anyone has any data on what a typical response rate for web surveys (the kind that are embedded on a site, like the one here on IXDA or the exit site variety)?  If anyone's implemented them before, what information were you hoping to discover and how useful did you find the results? 

Also looking for any information about how to best implement them on a site in terms of visibility, encouraging participation, etc.

you should check on Google +'s feedback.  It's feedback taken to the next level.  It may not be the best way to implement it, but I think it's rather useful, instead of just having user typing contxtual feedbacks.  


I'm also interested in the effectivness of the feedback button because I recently had to design one for a site, and since product is beta, not much analytics regarding the usage and effectivness of the feedback button.
