How to consult as an IxDA?

I was recently asked how to get started doing contract/consulting work
as an IxDA (what type of business entity to form, what standard agreements to
use, etc.).

Does anyone know of a good reference site for this type of information? Any good pointers? I know this has been discussed before but I've never paid that much attention to it.
