Name Top Agile UX Consulting Firms

Hello all,

I'm looking for a "short list" of top consulting firm recommendations (Boston, NY area preferred, but US-wide is ok too) to consider from anyone who has had direct experience working with such firms that have really proven their worth to introduce and foster transormative work practice around Agile UX for web design/development teams. To help frame the scope of the consulting, I'm envisioning a 5-8 day cross-discipline/stakeholder workshop and educational service focused on agile UX and collabortive web product design/innovation.

How to consult as an IxDA?

I was recently asked how to get started doing contract/consulting work
as an IxDA (what type of business entity to form, what standard agreements to
use, etc.).

Does anyone know of a good reference site for this type of information? Any good pointers? I know this has been discussed before but I've never paid that much attention to it.
